Established in 2002 by Louise Chance, A Chance For Life Ltd has swiftly achieved an excellent reputation as a reliable, cost effective and innovative service provider, operating throughout Lancashire, Cumbria and the North of England.
Services primarily fall in the category of healthcare, although Occupational Therapy and Case Management intervention often encompasses a variety of needs such as social care, physical disability, mental health, vocation and employment.
How our services are delivered
Telephone/email enquiry received. Relevant Professional consulted. Letter/email providing CV of professional and outline of costs.
Exchanged and agreed.
On receipt of written instructions, the allocated ACFL professional will review any medical and other reports provided by the instructing party. A home visit will be arranged with the client in order to undertake detailed assessment of needs.
Home visit carried out. Report including recommendations and costs will be sent to instructing parties.
Agreement of recommendations. Service commenced.
