Our Case Managers empower parents to support their children and young people to be as involved in their rehabilitation as they are able.
Children and young people are placed at the centre of their goals. Parents and other people who are important to the child or young person, such as teachers, carers, respite teams and extended family, are encouraged to be involved with the rehabilitation programmes and reviews.
Their rehabilitation team is coordinated by the Case Manager to ensure every child and young person meets their developmental and educational goals and to support transition to adulthood and independence.
Our Case Managers have extensive networks to local, regional and national specialists in rehabilitation, posture management, sensory integration, mobility, language development, healthcare and education, to give every child and young person the greatest opportunity to meet their potential.
