Set Goals

Our team strive to be dynamic and innovative in their approach and we involve clients, and where appropriate, their families,  in decision making about their rehabilitation and support.

We set short and long term goals that are individualised and meaningful to the client, thus enabling them to reach their full potential and maximise their quality of life. We re-evaluate these goals regularly and use this to demonstrate progress in rehabilitation.

 

For Adults

We focus on your needs and will work with you, and those important to you, to find ways to achieve your goals, whether that is at home, in the wider community, socially, educationally or for work.

We will help you to live a life as independently as you can; we can help you to find the best rehabilitation team, ensure you are in the right accommodation, and help you to find the right support team to help you achieve your life goals. We work sensitively to provide you with the correct level of support, when you need it.

For Children and Young People

Our Case Managers empower parents to support their children and young people to be as involved in their rehabilitation as they are able.

Children and young people are placed at the centre of their goals. Parents and other people who are important to the child or young person, such as teachers, carers, respite teams and extended family, are encouraged to be involved with the rehabilitation programmes and reviews.

Their rehabilitation team is coordinated by the Case Manager to ensure every child and young person meets their developmental and educational goals and to support transition to adulthood and independence.

Our Case Managers have extensive networks to local, regional and national specialists in rehabilitation, posture management, sensory integration, mobility, language development, healthcare and education, to give every child and young person the greatest opportunity to meet their potential.
